Registration

I hereby permit my child to participate in all CTEEN activities, and to join in trips on and beyond Chabad Lubavitch of Idaho properties and use any transportation selected by the Chabad Jewish Center.

Permission is hereby given to use in promoting the program and in other ventures directly relating to the program (i) digital, photographic and video images or likenesses of participant; audio of participant; and (ii) statements, articles, names, music, art, photographs, audio recordings, films and videos created by participant or originating from Chabad Jewish Center or from a program-related activity.

I further release and agree to indemnify and hold harmless Chabad Lubavitch of Idaho, Chabad Jewish Center and its officers, servants, volunteers or assigns from any liability concerning my child’s involvement in the program and further agree that the use of any premises during the program is made at the risk of the registrant.

I understand and agree that Chabad Jewish Center reserves the right to dismiss, in its sole discretion, any participant whose condition, conduct, influence or behavior is deemed unsatisfactory or detrimental to the best interests of the program or fellow participants or who violates program rules and regulations.  In the event of dismissal, fees paid will be forfeited. 

I have read and agree to all of the terms and conditions in this Registration Form.  I am including a non-refundable $75.00 registration fee per participant along with submission of this form.
